A hospital management simulation with made-up diseases and bizarre-looking equipment.

Theme Hospital is a management simulation in which the player builds and runs a hospital, but with one significant twist: the patients suffer from completely made-up ailments, such as Bloaty Head or Alien DNA, and treatment takes place in machines that look like they escaped from a grotesque circus. That satirical layer, however, doesn't disguise the fact that a very serious operations simulation runs underneath it. The player places consulting rooms, waiting areas, laboratories, and operating theaters, hires doctors, nurses, handymen, and cleaners, and keeps watch over the budget, treatment prices, reputation, and the satisfaction of both staff and patients.

The gameplay is built on constantly resolving bottlenecks. A patient first arrives at reception, then goes to a consulting room where a diagnosis is made, possibly followed by a series of tests, and only then to treatment, so any missing room instantly blocks the whole flow and queues start forming outside the doors. On top of this the game adds epidemics that must be hushed up or handled before a fine arrives, inspector visits, earthquakes, equipment breakdowns, and research that unlocks new equipment and improves what's already there. Individual levels have clear goals and gradually ramp up the challenge. Visually it's classic isometric graphics with a huge number of small animations, and the sound is unmistakable, especially the hospital PA announcements, which comment on events with dry humor and became the game's signature trademark.

At the time it was a very well received title that built on the studio's older management tradition and confirmed that operations management could be entertaining even for players who don't love spreadsheets. Its influence remains visible today, a whole range of modern operations simulations openly cite it as an inspiration, and one of them was created directly as its spiritual successor. The original game is still playable thanks to an open engine that runs it in high resolution on today's systems, and its pace and humor hold up without losing a beat.
