A survival horror about an inmate condemned for a murder he can't remember; a prison full of monsters.

The Suffering from 2004 is a survival horror game by Surreal Software, in which the player takes on the role of Torque, an inmate sentenced to death for the murder of his own family — a crime he himself doesn't remember, and whose truth remains unclear for much of the game. Shortly after his sentence begins, a supernatural massacre breaks out at the prison on Carnate Island, and the facility turns into a hell full of monsters born from the dark history of violence that took place on the island over the previous decades of its operation.

The gameplay combines a third-person shooter with action-RPG elements — Torque collects weapons, upgrades his character using found items and experience, and at key moments can transform into a monster and fight with brutal physical attacks instead of firearms. A key feature is the morality system: the player's decisions and behavior during the game (aggressive or, conversely, merciful toward other characters) affect not only Torque's psyche and appearance, but also whether he actually committed the crime he was convicted of, leading to several different endings.

The game stood out for its dark, unsettling prison and industrial atmosphere, with an emphasis on psychological horror rather than cheap jump scares. It was praised for its bold moral choices and ambiguous story without clear answers to the question of the hero's guilt. The Suffering is among the titles that showed survival horror on the PS2 could work even in a more action-oriented, shooter-based form, and it still has a loyal circle of fans who appreciate its harsh tone and unconventional storytelling. The sound design, with prisoner voices, metallic screeching, and wailing, heightens the claustrophobic atmosphere and is among what players remember most about the game today. The prison and surrounding buildings are also littered with findable documents and recordings that gradually piece together the island's dark history.

Why play it: For fans of survival horror in a more action-oriented, shooter-based form, with the claustrophobic atmosphere of a prison and moral decisions without clear-cut answers about the hero's guilt.
